How do I get a MAXTERRA Sample Kit

Here are the steps to receive your Sample Kit

1. On every nexgenbp.com webpage, you will see an Orange button in the upper right-hand corner that says Get Samples. Click this button
   
   
2. A pop-up form will appear that asks for the information NEXGEN needs to ship you your Sample Kit.
3. Please fill out the required fields.  (Note: all fields are required, sorry)
4. Hit the Submit button at the bottom of the form
5. Your information is sent to the NEXGEN team to fulfill your Sample Kit
6. You will receive an email confirming that your Sample Kit request has been received.
